Physical Requirements and Information

 

• All physicals must be completed by a doctor on the GHSA physical form

  • GHSA form is available below

 

• On the first day of spring practice each player must have a NEW physical that does not expire until at least November of this year.

 

• Each April HHS will host physicals on campus for all athletes. Exact dates and times will be sent out each year leading up to the physical date.

  • Cost is $25 and checks made out to HHS or pay via Venmo to Gridiron Club

  • The athlete must arrive with a GHSA physical form with the history portion completed and signed by parent or guardian as well as with payment (or prior payment)

 

• You may use your family doctor for physicals just make sure it is completed on the GHSA physical form.
